Anonymous Submissions
Some public forms allow anonymous submissions. These forms are designed so that you can share input without providing your name or contact information, if you prefer.
Anonymous Forms
For forms that support anonymous submissions:
- No identifying information is required
- The portal stores only the text or responses you submit
- They are used for open feedback where your identity is optional
Identified Forms
For forms that request your name or contact information:
- Providing this information is voluntary
- Your details are used only for the purpose of responding to or following up on your submission
- Your information is not shared outside UUCF leadership
Whenever possible, you may choose whether to identify yourself or to submit feedback without including your name.
