Public vs Member-Only Forms

UUCF uses a variety of online forms to gather feedback, coordinate volunteers, and communicate with our community. Some forms are available to anyone, while others require a Member Portal account.

Public Forms (Anyone May Submit)

These forms do not require an account:

  • Surveys
  • Feedback forms
  • Volunteer interest forms
  • Public event or program sign-ups

Public forms are used when UUCF wishes to hear from:

  • Members
  • Friends
  • Visitors
  • Community partners
  • The wider public

Anyone can use public forms, including people who do not have a Member Portal account.

Member-Only Forms

Some forms require you to log in to the Member Portal. These may include:

  • Annual pledge forms
  • Time & Talent commitments
  • Directory settings
  • Member feedback that requires follow-up tied to your account
  • Forms involving confidential or administrative information

Member-only forms help UUCF ensure accuracy, security, and direct communication with individuals about their submissions.
